Telemedicine/Telehealth disclosure

IF YOU ARE CURRENTLY COVERED UNDER A MEDICAL PLAN THAT IS NO LONGER OFFERED BY GUARDIAN, you may be eligible for Telehealth services under your current policy. Telemedicine or Telehealth Services include medical consultations with your health care provider that occur by video- or web-conference, telephone conference, text message and email. If you receive Telehealth services, you or your health care provider may submit a claim to Guardian, using the standard claim form and procedure.
